Enquire to be a new patient

Please email us at nsoccg.TLSenquiries@nhs.net to request a registration form and let us know your postal address so we can check you’re in our catchment area.

When we have received your registration request we will first check that you are within our catchment area before registering you as a patient. Please be aware that sending us a registration request does not automatically mean we are able to register you here. If you have any questions please don’t hesitate to get in contact with us via email at nsoccg.TLSenquiries@nhs.net or phone us on 01934 622665.

Please print out a registration form and new patient questionnaire (see link below), or alternatively you can come into the surgery and fill out the forms in person.

Download and print our Registration Pack which includes the new patient questionnaire and also download and complete the NHS Family Doctor Services registration form. When you have filled in these two forms, please return them to reception.

Improved Access

We are working together with other local practices to offer patients access to more appointments in the early mornings, evenings and at weekends. To find out more about these services, please contact the practice.

Patient update from Tudor Lodge Surgery:

Like other GP practices, we are affected by the current national shortage of blood tubes.

In order to continue blood tests for people with the greatest clinical need, we are delaying routine and non-urgent blood tests in line with national guidance.

If you have an appointment booked in the next few weeks the reception team will be in touch via phone or text. Your appointment will be rescheduled once the situation is resolved.

We will be providing regular updates on our website, which we encourage you to check for the latest info.

Please don’t contact us – we will be in touch with you when appointments can be rescheduled. Thank you for your patience and cooperation.
