Please email us at nsoccg.TLSenquiries@nhs.net to request a registration form and let us know your postal address so we can check you’re in our catchment area.
When we have received your registration request we will first check that you are within our catchment area before registering you as a patient. Please be aware that sending us a registration request does not automatically mean we are able to register you here. Please print out a registration form and new patient questionnaire (see link below), or alternatively you can come into the surgery and fill out the forms in person.
Download and print our Registration Pack which includes the new patient questionnaire and also download and complete the NHS Family Doctor Services registration form. When you have filled in these two forms, please return them to reception.
